How do I know which case I have?

If your tank is marine, the same white-spot appearance points to C. irritans, but first rule out marine velvet (Amyloodinium ocellatum), which looks superficially similar and is far more lethal.
The visual difference matters enormously. Ich spots are oval to round, relatively large (roughly 0.5 to 2 mm), and sparse enough to be counted individually. Velvet spots are far smaller (10 to 80 micrometers), so numerous that they give the fish a dusty or powdered-sugar appearance, and often appear golden, tan, or rust-colored rather than white over a wide surface area. Velvet attacks the gills first and may kill fish before any body coating is visible. If your fish are breathing rapidly or dying suddenly with minimal visible spotting, treat for velvet rather than ich. Misdiagnosis leads to the wrong treatment and potential loss of fish.
How to treat freshwater ich step by step
Step 1: Raise the temperature gradually. For tropical fish species, slowly increase the aquarium temperature to roughly 29 to 30 degrees C (84 to 86 degrees F), raising it no more than 1 to 2 degrees C every few hours to avoid stressing your fish. Add supplemental aeration immediately, because warm water holds less dissolved oxygen. At around 30 degrees C, many ich strains cannot reproduce, and tomont cysts release roughly 42% fewer theronts than at cooler temperatures. Do not use this approach for coldwater or sub-tropical species such as goldfish, danios, rosy barbs, or loaches, as high temperatures compromise their immune function and health.

What about aquarium salt? Adding salt to a freshwater tank is used as an adjunct or standalone treatment, but this nuance depends on strain and situation; it is not a guaranteed standalone cure for all cases. Combining salt with elevated temperature and, for severe outbreaks, with formalin and malachite green increases reliability.
How to treat marine ich step by step
Marine ich must be treated in a dedicated quarantine tank. No currently available product sold as reef-safe has been shown to fully eradicate C. irritans from a system. You have three main options.
Option 1: Copper treatment. Copper-based medications such as ionic and chelated products (Cupramine, Copper Power, and similar) are the most established marine ich treatment. Copper kills the free-swimming theront and the protomont before it encysts. Maintain a verified therapeutic concentration monitored with a copper test kit throughout the treatment period. Because tomonts at typical reef temperatures (23 to 30 degrees C) can take 3 to 28 days to excyst, the widely accepted minimum is 30 days in a single quarantine tank. A safer margin is 42 to 45 days when fish can tolerate extended exposure. An alternative is 14 days of copper followed immediately by at least two weeks of observation in a sterile, copper-free quarantine tank. Copper must never be used in a display tank containing live rock, because it bonds to calcium carbonate and renders the dose uncontrollable. Copper is toxic to all invertebrates at any concentration.
Option 2: Tank Transfer Method (TTM). Move fish between two separate, clean, fully equipped tanks every 72 hours or less for a minimum of 12 days and at least four transfers. Trophonts detach and encyst as tomonts in the vacated tank, while the fish are moved to a clean tank before theronts can re-infect them. If velvet is suspected or cannot be ruled out, use a modified 36-hour transfer interval instead of the standard 72-hour interval, because velvet requires shorter transfer windows.
Option 3: Fallow the display tank. Remove all fish and leave the tank fishless for a minimum of 76 days. Because C. irritans is an obligate fish parasite, theronts that hatch will die of starvation within 24 to 48 hours without a host. The 76-day figure accounts for the longest documented tomont stage (roughly 72 days, an extreme occurring only in cooler water) plus a buffer for the theront stage. At normal reef temperatures (24 to 27 degrees C), the full cycle typically completes in 1 to 2 weeks, but 76 days is a conservative safety margin. Extend to 90 days or more if your tank has deep sand beds, areas under rocks, or canister filters, because tomonts can enter dormancy in low-oxygen zones and resume development once oxygen is restored. Invertebrates, corals, and other non-fish inhabitants may remain in the tank during fallow. Increase flow and oxygenation throughout the system and vacuum substrate to reduce anaerobic zones.
Hyposalinity (lowering specific gravity to approximately 1.009, roughly 12 to 14 parts per thousand) in a quarantine tank causes osmotic shock to C. irritans theronts, which cannot osmoregulate under low-salinity conditions. It must be applied over several weeks with gradual salinity restoration afterward. Note that hyposalinity does not treat velvet, brooklynella, or other marine parasites, and whether it is reliable against all strains of C. irritans depends on the situation; some strains may survive at very low salinities.
Does ich harm fish beyond the visible spots?
Yes. Trophonts embedded in the gills cause direct physical damage to gill tissue, which impairs respiration. Fish with gill-heavy infections may show rapid breathing and lethargy even before body spots are prominent. Velvet causes this problem far more severely and rapidly than ich, so any fish showing acute respiratory distress needs urgent assessment.
